You can submit your forms via email or fax, but they must have a wet (ink) signature to be processed. Electronic or digital signatures are not accepted at this time.
As a member of the Local 2041 Acoustic & Drywall Health and Welfare Plan, you will be able to access your benefits by submitting claims for medical expenses incurred.
You will be able to submit health claims electronically by logging into the Ellement App; however, some providers have the option to direct bill.
